Please read the following statement carefully before agreeing.

I understand that participating in physical training may involve a risk of injury or illness. I voluntarily assume all risks and release Royally Phit LLC, its trainers, and representatives from any liability related to my participation.

I agree to disclose any conditions, limitations, or medications that may affect my ability to exercise safely. I understand that:

• Sessions must be used within 30 days of purchase.

• 24-hour notice is required for cancellations to avoid loss of session.

• Late arrivals or no-shows may result in forfeiture of the session or 10 "Dru Hills" for each minute late.

• Package must be purchased before beginning training.

• A full refund may be issued before the first session only.

By submitting this form, I acknowledge that I accept full responsibility for my health and well-being, and I understand that Royally Phit LLC and its trainers assume no responsibility.

By signing this document, I acknowledge the inherent risks and voluntarily assume full responsibility for any injury, damage, or loss that may result from my participation in this activity. I hereby waive and release the business, its owners, and its staff from all liability, past, present, and future, relating to the services provided.
